Samsung Galaxy Tab 3: Oh, Another One?

Just in case you thought Samsung didn't offer enough variety in its tablet range, its gone ahead and launched yet another. This 7-inch Galaxy Tab 3 is brand new and, um, like virtually every other Samsung tablet that's gone before it.

The third iteration of Samsung's first tablet attempt, this little seven-incher packs a 1.2GHz processor, 8GB or 16GB of storage, 3-megapixel rear camera, 1.3-megapixel front facer, and Android 4.1. A fairly bland spec list to accompany what is quite a bland product.

The Wi-Fi version of the Tab 3 will roll out across the world in May, with a 3G model following in June. There's currently no word on pricing, but it will probably be comparable to the existing Tab 2. Not that you'd notice the difference. [Samsung via Engadget]

The Difference Between Patient-Centered Medical Homes and ...

patient-centered medical homeIn healthcare, it?s common to hear buzzwords thrown around. In the past we have given you a handy guide for navigating the tricky acronyms elder care professionals and caregivers frequently come across. ?

Given the complexity of innovation in the healthcare field, it?s sometimes tough to keep up the development of new care delivery models. Recently two community-based care models have garnered a great deal of attention: the patient-centered medical home (PCMH) model and the concept of Medicaid health homes.

While patient-centered medical homes and health homes share some similarities, there are key differences in how each model enhances care for those with chronic conditions and supports aging in place. We break those down for you here:

The Patient-Centered Medical Home (PCMH) Model

Proposed in 2007 by the American Academy of Family Physicians (AAFP), American Academy of Pediatrics (ACP), American College of Physicians (AAP), and American Osteopathic Association (AOA), the patient-centered medical home model aims to provide comprehensive, coordinated, and continuous care for all populations from children to seniors. It requires a team-based, physician-led approach that seeks to enhance the role of primary care and organize care around the patient. According to the Patient-Centered Primary Care Collaborative, clinicians practicing within the medical home model:

  • Assume responsibility for the ongoing care of patients and coordinate care over multiple settings
  • Are more accessible to patients by providing expanded hours, easier scheduling, and remote consultations by phone and email
  • Utilize electronic personal health records
  • Conduct regular checkups and encourage preventative care

Medicaid Health Homes

As of early 2011, the Patient Protection and Affordable Care Act (ACA) health reform law established ?health homes? as a Medicaid option to provide services specifically for beneficiaries with chronic conditions. Health homes are designed to a person-centered, integrated care model that coordinates medical care, behavioral health services, as well as community and social supports.

What Medicaid patients are eligible for participation in a health home?

  • Those with two chronic conditions
  • Those with one chronic condition and risk of a second
  • Those with one ?serious and persistent? mental health condition

According to the ACA, health homes must provide the following services:

  • Comprehensive care management
  • Care coordination and health promotion
  • Transitional inpatient to outpatient care
  • Individual and family support
  • Referrals to community and social support services
  • Services linked through health information technology

In many states the health home model builds upon the medical home model, expanding the linkages and breadth of services to support the needs of those with chronic illnesses. The goal of the Medicaid home health model is to improve clinical outcomes and overall healthcare quality for persons with long-term conditions, as well as reduce per-capita healthcare expenditures by delivering more effective, coordinated care. Unlike the PCMH model, States have flexibility to determine eligible health home providers. The provider may be a designated professional such as health clinic or home health agency or a team of health professionals which may include mental health workers, dieticians, nurses, and pharmacists.

Musical of 'Rocky' heading to Broadway

NEW YORK (AP) ? It's been a knockout in Germany. Now Sylvester Stallone hopes a musical based on his beloved boxing film "Rocky" will also be a hit on Broadway.

Producers said Sunday they plan to get "Rocky" up and punching at the Winter Garden by February following a successful debut in Hamburg last fall.

Based on the Oscar-winning 1976 film, the musical features a score by "Ragtime" veterans Stephen Flaherty and Lynn Ahrens, and a story by Thomas Meehan, who wrote "The Producers" and "Hairspray."

Originally written in English but translated into German for its world premiere and billed as "Rocky: Das Musical," the show is produced by Stallone and Stage Entertainment USA.

"The reason I think it has worked so well there and why I think it'll work on Broadway is that, yes, it's a story about boxing, but the real story is actually an intimate, powerful and gritty and moving love story between two people who are both lonely and in a difficult place in their worlds," said Bill Taylor, managing director of Stage Entertainment USA. "They rescue each other. It's very uplifting."

The musical stays close to the film, which charted the rise and romance of amateur boxer and debt collector Rocky Balboa, played in Germany by Drew Sarich. No casting has been decided for New York.

In the story, Balboa, nicknamed the Italian Stallion, gets his shot against undefeated heavyweight champion Apollo Creed, played in the film by Carl Weathers. He also woos a love interest, Adrianna "Adrian" Pennino. Stallone wrote the screenplay and it won the best picture Oscar in 1976.

The film made famous the image of Balboa running up the steps of the Philadelphia Museum of Art and the quote "Yo, Adrian!" The German production kept the trumpet-laden funky theme "Gonna Fly Now" and the anthem "Eye of the Tiger," written for "Rocky III." Both will also be in the Broadway version.

The director is Alex Timbers, who directed Broadway's "The Pee-wee Herman Show" and directed and wrote the book for "Bloody Bloody Andrew Jackson." The boxing choreography is being done by Steven Hoggett, who choreographed "American Idiot," ''Peter and the Starcatcher" and "Once."

"This is not boxers doing a kick line," Taylor said. "It's stunning movement representing some of the sparring and the fighting. It's very, very cleverly created."

"Rocky" will be the first new tenant at the Winter Garden Theatre in years. The show that's been there since 2001, "Mamma Mia!," is transferring to another Broadway venue.

The musical will follow two other boxing-related works to appear on Broadway recently: Mike Tyson's one-man show about his life in and out of the ring, and a revival of Clifford Odets' "Golden Boy" about a young man torn between his natural talent as a violinist and the fast money of boxing.

Bangladesh building collapse death toll nears 350

SAVAR, Bangladesh (AP) ? Police in Bangladesh took five people into custody in connection with the collapse of a shoddily-constructed building that killed at least 348 people, as rescue workers admitted Saturday that voices of survivors are getting weaker after four days of being pinned under the increasingly unstable rubble.

Still, in a boost for the rescuers, 29 survivors were pulled out Saturday, said army spokesman Shahinul Islam.

Most of the victims were crushed by massive blocks of concrete and mortar falling on them when the 8-story structure came down on Wednesday morning -- a time many of the garment factories in the building were packed with workers. It was the worst tragedy to hit Bangladesh's massive garment industry, and focused attention on the poor working conditions of the employees who toil for $38 a month to produce clothing for top international brands.

Among those arrested Saturday were two owners of a garment factory, who a Dhaka court ruled can be questioned by police for 12 days without charges being filed. Also detained are two government engineers and the wife of the building owner, who is on the run, in an attempt to force him to surrender. Violent public protests continued sporadically in Dhaka and spread to the southeastern city of Chittagong where several vehicles were set on fire.

Working round-the-clock since Wednesday through heat and a thunderstorm, rescuers on Saturday finally reached the ground floor from the top of the mountainous rubble through 25 narrow holes they have drilled, said Brig. Gen. Ali Ahmed Khan, the head of the fire services.

"We are still getting response from survivors though they are becoming weaker slowly," he said, adding that rescue workers were now able to see cars that were parked at the ground level.

"The building is very vulnerable. Any time the floors could collapse. We are performing an impossible task, but we are glad that we are able to rescue so many survivors." He said the operations will continue overnight as chances fade of people surviving for a fifth day with possibly grievous injuries and the heat.

The building site was a hive of frenzied activity all day with soldiers, police and medical workers in lab coats working non-stop. Rescuers passed bottles of water and small cylinders of oxygen up a ladder leaning against the side of the building to be given to possible survivors inside.

They used bare hands and shovels, passing chunks of brick and concrete down a human chain away from the collapsed structure. On the ground, mixed in the debris were several pairs of pink cotton pants, a mud-covered navy blue sock and a pile of green uncut fabric.

Nearby, Abul Basar wept as he awaited news of his wife, who worked in one of the garment factories. "My son says that his mother will come back some day. She must return! " he cried.

Every once in a while a badly decomposed body would be brought out, covered in cloth and plastic, to a spot where ambulances were parked. Workers furiously sprayed air-fresheners on the bodies to cover the stench, leaving the air thick with the smell of death and cheap perfume.

The bodies were kept at a makeshift morgue at the nearby Adharchandra High School before being handed over to families. Many people milled around at the school, waving photos of their missing loved ones.

Junior local government minister Jahangir Kabir Nanak put the death toll at 348. Military spokesman Shahinul Islam said 2,429 survivors were accounted for, including 29 who were pulled out Saturday.

Junior Home Minister Shamsul Haque Tuku said police had arrested Bazlus Samad, managing director of New Wave Apparels Ltd., and Mahmudur Rahman Tapash, the company chairman. He told reporters that police had also detained the wife of Mohammed Sohel Rana, the owner of the collapsed Rana Plaza building, for questioning. The top three floors of the eight-story building were illegally constructed.

Authorities are still searching for Rana, a local politician, who hasn't been seen publicly since the building collapsed. Negligence cases have been filed against him. Police in Bangladesh often detain relatives of missing suspects as a way to pressure them to surrender.

Dhaka police superintendent Habibur Rahman said Rana was a local leader of ruling Awami League's youth front. His arrest, and that of the factory owners, was ordered by Prime Minister Sheikh Hasina, who is also the Awami League leader.

Police said they detained for questioning two engineers working for the Savar municipality, Imtemam Hossain and Alam Ali. They did not say what role they played in approving the design of the building but it was clear that the arrests amounted to a widening crackdown. They spoke on condition of anonymity because they are not authorized to speak to the media.

A garment manufacturers' group said the factories in the building employed 3,122 workers, but it was not clear how many were inside it when it collapsed.

Police say they ordered an evacuation of the building on Tuesday after cracks in Rana Plaza were found, but the factories ignored the order and were operating when it collapsed the next day. Video before the collapse shows cracks in walls, with apparent attempts at repair. It also shows columns missing chunks of concrete and police talking to building operators.

Officials said soon after the collapse that numerous construction regulations had been violated.

The disaster is the worst ever for the country's booming and powerful garment industry, surpassing a fire five months ago that killed 112 people and brought widespread pledges to improve worker-safety standards. Since then, very little has changed in Bangladesh, where low wages have made it a magnet for numerous global brands.

Bangladesh's garment industry was the third largest in the world in 2011, after China and Italy, having grown rapidly in the past decade. The country's minimum wage is the equivalent of about $38 a month.

Among the garment makers in the building were Phantom Apparels, Phantom Tac, Ether Tex, New Wave Style and New Wave Bottoms. Altogether, they produced several million shirts, pants and other garments a year.

The New Wave companies, according to their website, make clothing for several major North American and European retailers.

Britain's Primark acknowledged it was using a factory in Rana Plaza, but many other retailers distanced themselves from the disaster, saying they were not involved with the factories at the time of the collapse or had not recently ordered garments from them.

Wal-Mart said none of its clothing had been authorized to be made in the facility, but it is investigating whether there was any unauthorized production.

Slow Insurance Approvals Strand Mental Health Patients in ER A significant problem regarding health care access involves obtaining hospital admission for those experiencing a mental health crisis.

The backlog in obtaining insurance approval for admission to a hospital bed is problematic for the patient in crisis, other emergency room patients and physicians.

A research letter to be published in the May issue of the journal Annals of Emergency Medicine argues that pre-authorization process is akin to health care ?rationing by hassle factor.?

?An emergency department is just about the worst place for a psychiatric patient to wait for an inpatient bed, and yet that is exactly what the pre-authorization process forces on millions of these vulnerable people,? said senior author J. Wesley Boyd, M.D., Ph.D.

?The thousands upon thousands of hours emergency physicians spend obtaining prior authorization for admission to the hospital are hours we are not spending on direct patient care. Only Medicare does not require prior authorization for us to admit psychiatric patients to the hospital; maybe they are onto something.?

In the study, researchers recorded data on 53 patients ? most were in the emergency department because they were having suicidal thoughts.

Half of the authorization requests took under 20 minutes to be approved, but 10 percent of the patients? authorizations took an hour or more. Only one of the 53 patients? insurance carriers denied pre-authorization. There are approximately 2.5 million psychiatric admissions to hospitals every year in the U.S.

?Psychiatric care is really the poor stepchild in the world of insurance coverage,? said lead author Amy Funkenstein, M.D., of Brown University in Providence, R.I..

?Insurance carriers reimburse poorly and as a consequence, hospitals often have inadequate resources for patients who urgently need this care. The situation is so dire that ERs are now being designed and configured to house psychiatric patients awaiting placement as inpatients. These patients deserve better.?

Sequester: Still a Degree of Uncertainty for Higher Education

According to College Admissions Assistance, LLC, although there is daily speculation about how sequestration will affect different domestic programs, there is much uncertainty among higher education officials.

According to College Admissions Assistance, LLC (CAA), planning ahead for an unknown circumstance is the challenge colleges and universities have been facing for months. Now that budget cuts are starting to be defined for at least this fiscal year, higher education officials are beginning to move ahead with what they know, but that information is still limited.

Even with daily speculation in the news about how sequestration will affect different domestic programs, there is much uncertainty among higher education officials. The uncertainty stems from a few factors: what does an across-the-board cut mean for education programs, which programs will be affected, how will the cuts be applied, and how will the yet-to-be-approved government spending plan affect future programs?

So, how can you plan for college without knowing how long these cuts will last or how they will affect the college-bound student? According to College Admissions Assistance (CAA), an educational-services company that provides a college planning program for high school students, the answer is to understand the facts and keep your options open.

A few facts are clear. In response to the sequester laws put in place by the Balanced Budget and Emergency Deficit Control Act of 1985, the Department of Education announced the following (http://studentaid.ed.gov/about/announcements/sequestration):

-The Pell Grant is exempt from the cuts for the 2013-2014 year.

-Campus based aid, specifically the Federal Work-Study and Federal Supplemental Educational Opportunity Grant (FSEOG) programs, is expected to be cut but official numbers have not been given.

-Loan origination fees are due to increase by a small amount. Current fees for Federal Stafford Direct Subsidized and Unsubsidized Loans are at 1 percent and will increase to 1.051 percent. Parent PLUS Loan fees will increase from 4 percent to 4.204 percent.

-For the 2013-2014 school year, individual Teacher Education Assistance for College and Higher Education (TEACH) Grants will be cut by 12.6 percent and individual Iraq-Afghanistan Service Grants will be cut by 37.8 percent.

Some universities will be affected more than others depending on how much they rely on government funding. Colleges may choose to use their own institutional funding to make up for any federal shortages or ?tighten their belts? in other areas. Clif Smart, president of Missouri State University said in the March 19, 2013 edition of The Standard, ?We have reserves to weather any short term loss of funding, so we should be fine this year.?

So, the question now is how do you plan for college when so much is still uncertain?

CAA advises all students to apply to at least four colleges with at least one in-state public institution as their ?financial safety? school.

?The concept of finding at least one ?financial safety? school has never been more important, but to do that, parents need a basic understanding of how financial aid works and how it is awarded,? said Brenda Watkins, Executive Director of Student Services for CAA. ?Parents can sabotage their own efforts to get the best award possible by having their student apply to only one college or applying to colleges without determining the proper fit on an academic, social, emotional, and financial level.?

Watkins explains that students who fit on all four levels are much more attractive candidates, which may translate into higher institutional funding. However, she said those who applied to colleges without regard to ?fit? still have options.

For students who did not have access to this type of information, Watkins advises they carefully review all award notifications as they arrive and apply to additional colleges that are still accepting applications, as needed. CAA advises students and parents to consider the four-year cost of the colleges along with any financing options.

?Exhaust all federal loan options before considering risky alternatives such as private loans, home equity loans, or borrowing from retirement plans,? said Watkins. ?Borrowing from your retirement plan in this volatile economy is unwise because you lose tax benefits, earnings, and face stiff penalties if you cannot repay the debt within five years.? Yes college is expensive, but it is still one of the best investments you can make. Recently, free-agent linebacker Brendon Ayanbadejo has been backpedaling more than a cornerback.

After suggesting this his support for same-sex marriage was a factor in his release by the Ravens, Ayanbadejo quickly said it wasn?t.? Now, not long after explaining that four gay players are considering coming out at the same time, Ayanbadejo says they aren?t.

Via Outsports.com, that was the upshot of Ayanbadejo?s Friday night appearance with Anderson Cooper of CNN.

?No, actually, what it is is, is there are organizations I?m in contact with, and there are individuals I?m in contact with and collectively we know of some gay players,? Ayanbadejo said when asked if he knows the names of the four gay players who are considering coming out.? ?And these players, some of them are anonymous, some of them we know who they are, but their identity is super secret and nobody wants to reveal who they are, and some of them don?t want to reveal who they are, rightfully so because it?s entirely up to them what they are going to do.

?What we want to facilitate is getting them all together so they can lean on each other, so they can have a support group.? And potentially it?s possible, it?s fathomable, that they could possibly do something together, break a story together.?

Apart from Ayanbadejo?s chronic inability to get his facts straight, he has an increasingly obvious desire to see one or more gay players come out. ?Gay players wrestling with this intensely personal decision easily could perceive that they are merely pawns in a much broader agenda being driven by straight players and/or members of the media who believe that the time has come for closeted gay players to be openly gay.

As former NFL player Wade Davis, who is openly gay, tells Outsports.com, ?The problem is that you have straight people speaking on the behalf of these closeted gay athletes, instead of letting the gay athletes speak for themselves.?

There?s a fine line between providing support and applying pressure.? While having multiple gay players come out at once would reduce the distraction created by one gay player coming out, if none of them choose to come out, that?s their business.

Ultimately, we believe people should be allowed to be who they are, and who they choose to be.? That includes straight, gay, and closeted.? If a gay player simply doesn?t want to come out, that?s his prerogative ? and no one gay or straight should be trying to get him to do something he doesn?t want to do.

Chinese foreign fisheries catch 12 times more than reported, study shows

Apr. 3, 2013 ? Chinese fishing boats catch about US$11.5 billion worth of fish from beyond their country's own waters each year -- and most of it goes unreported, according to a new study led by fisheries scientists at the University of British Columbia.

The paper, recently published in the journal Fish and Fisheries, estimates that China's foreign catch is 12 times larger than the catch it reports to the United Nation's Food and Agriculture Organization, an international agency that keeps track of global fisheries catches.

Using a new method that analyzes the type of fishing vessels used by Chinese operators around the world and their catch capacity, the UBC-led research team estimates Chinese foreign fishing at 4.6 million tonnes per year, taken from the waters of at least 90 countries -- including 3.1 million tonnes from African waters, mainly West Africa.

"China hasn't been forthcoming about its fisheries catches," says Dirk Zeller, Senior Research Fellow with UBC's Sea Around Us Project and the study's co-author. "While not reporting catches doesn't necessarily mean the fishing is illegal -- there could be agreements between these countries and China that allow fishing -- we simply don't know for sure as this information just isn't available."

"We need to know how many fish have been taken from the ocean in order to figure out what we can catch in the future," says Daniel Pauly, principal investigator of UBC's Sea Around Us Project and the study's lead author.

"Countries need to realize the importance of accurately recording and reporting their catches and step up to the plate, or there will be no fish left for our children."

Potential map to more effective HIV vaccine

Apr. 3, 2013 ? By tracking the very earliest days of one person's robust immune response to HIV, researchers have charted a new route for developing a long-sought vaccine that could boost the body's ability to neutralize the virus.

The research team, led by Barton F. Haynes, M.D., director of the Duke Human Vaccine Institute, and John Mascola, M.D., acting director of the NIH Vaccine Research Center, have for the first time described the co-evolution of antibodies and virus in a person with HIV whose immune system mounted a broad attack against the pathogen.

Findings are published April 3, 2013, in the journal Nature.

Most vaccines work by inducing this antibody response, but the HIV virus has proved to be a difficult vaccine target. When HIV antibodies are produced, they typically have a limited range, and the virus changes rapidly to escape harm, leading to an arms race that the virus usually wins.

The current research was aided by new technologies that can detect early infection and track the subsequent immune response and virus evolution. It fills gaps in knowledge that have impeded development of an effective vaccine for a virus that has killed more than 30 million people worldwide.

"This project could only have been carried out by a multidisciplinary team working closely together," said Haynes, who led the work as a project of the Duke Center for HIV/AIDS Vaccine Immunology-Immunogen Discovery (CHAVI-ID) consortium, which is funded by the National Institute of Allergy and Infectious Diseases. "For the first time, we have mapped not only the evolutionary pathway of the antibody, but also the evolutionary pathway of the virus, defining the sequence of events involved that induce the broadly neutralizing antibodies."

The key to this finding was a person in Africa whose HIV infection was detected so early that the virus had not yet mutated to avoid the immune assault. The individual also exhibited a fortuitous trait that occurs in only about 20 percent of people infected with HIV -- an immune system that produces broadly neutralizing antibodies. These immune weapons attack vulnerable sites of the virus that are conserved despite mutations. In identifying the early viral infection, the team found the outer envelope, the viral surface glycoprotein, which triggered the start of the broadly neutralizing antibody development.

By tracking the precise virus and antibody pathways involved, the Duke CHAVI-ID and NIH teams now have a detailed road map for development of a potential vaccine, which involves immunogens with an outer envelope specifically selected to stimulate the production of broadly neutralizing antibodies.

"The next step is to use that information to make sequential viral envelopes and test them as experimental vaccines," Haynes said. "This is a process of discovery and we've come a long way with regard to understanding what the problem has been."

Sony Xperia ZL goes on sale in Germany

After going on sale in Canada yesterday, Germany now gets a chance to order Sony's premier smartphone, as the Xperia ZL is currently shipping from Phonehouse.de in black only. Those looking for a red or white version of this phone will have to wait a little longer, as is stock listed as "available soon." The phone is priced at €549,99, but can also be purchased with various subsidy prices on contract.

Potential buyers only have one option at present, because Phonehouse.de is the only retailer taking orders for the ZL at this time. Amazon.de has no launch date listed on their site, and Cyberport.de is reporting they will start taking orders and shipping phones in May.

While we here in the states are still waiting for our launch, time can be well spent looking over a close relative of the Xperia ZL, the Xperia Z, in our detailed review. Be on the lookout for a our review of the Xperia ZL, in the near future.

One does not simply pull an iPhone whole from the void and birth it in a single day. As anyone involved in the design, development, and manufacturing of iPhone-class products will tell you, it takes a long, long time and roadmaps typically stretch out over years. So, it comes as little surprise that the next two generations of iPhone, likely including an S-type update to the current iPhone 5, began under the direction of Steve Jobs. Still, given Jobs legacy, hearing it reaffirmed again seems to make many people happy. Which brings us to Mike Aldrax's report in the San Francisco Examiner:

[District Attorney George Gasc?n] said the next two generations of iPhones have already been developed.

?They preceded Tim Cook,? the district attorney said he was told of the future iPhones.

The reason a San Francisco district attorney is being sourced is because of his office's ongoing investigation into stolen mobile devices.

To say that Steve Jobs having a hand in these products isn't important is saying his involvement with Apple wasn't important, which is lunacy. Someone of his universe-denting talents having a hand in anything is amazing. However, Jony Ive on design and Tim Cook on logistics, Bob Mansfield on radios and chips, and the rest of the team no doubt contributed from the beginning as well, and will be there to adapt them to ever changing market conditions, and eventually shepherd them onto store shelves.

Eventually the ball gets handed off, and the team runs with it. That's how this stuff works. Still, it's nice to know Steve Jobs was there for these products, at least in the beginning, right?

Stocks slide with slowdown in manufacturing growth

Stocks opened April on a weak note, ending slightly lower after an industry group reported that US manufacturing growth cooled in March.?Industrial stocks fell the most in the S&P 500.

The stock market got off to a slow start in April, edging lower after the Standard and Poor's 500 index eclipsed its all-time high last week.

The main catalyst was a slowdown in U.S. manufacturing growth last month. The decline in the Institute for Supply Management's benchmark manufacturing index for March was worse than economists had forecast. Stocks started falling shortly after the report came out at 10 a.m. and stayed lower the rest of the day.

The Dow Jones industrial average closed 5.69 points, or 0.04 percent, lower at 14,572.85. The Standard & Poor's 500 index dropped 7.02 points, or 0.5 percent, to 1,562.17.

Industrial companies fell 1 percent, the most in the S&P. 3M, which makes Post-it notes, industrial products and construction materials, fell 66 cents, or 0.6 percent, to $105.65. Caterpillar, a maker of construction and mining equipment, dropped $1.33, or 1.5 percent, to $85.64.

Investors have raised their expectations for the U.S. economy as the market has climbed this year, said JJ Kinahan, chief derivatives strategist at TD Ameritrade. The Dow is up 11.2 percent in 2013, the S&P 9.5 percent.

"The numbers have to be outstanding in order to drive the market higher," Kinahan said. "It's a different mindset when we're at these levels."

The S&P 500 closed the first quarter at an all-time high of 1,569.19, surpassing its previous record close of 1,565.15 set on Oct. 9, 2007. The index has recaptured all of its losses from the financial crisis and the Great Recession. The Dow broke through its previous all-time high March 5.

The market has risen this year because of optimism that housing is recovering and that employers and starting to hire again. Strong company earnings and continuing stimulus from the Federal Reserve have also increased demand for stocks.

Small stocks fared worse than large ones Monday.

The Russell 2000, a benchmark of small-company stocks, fell 1.3 percent to 938.78, paring its gain for the year to 10.5 percent. It was the index's biggest decline in more than a month. The Nasdaq composite fell 28.35 points, or 0.9 percent, to 3,239.17.

April is historically the second-strongest month for stocks, Deutsche Bank analysts said in report released Monday. The S&P 500 has gained an average of 1.4 percent in April, based on returns since 1960, making it the second strongest month after December.

The last meaningful setback for stocks started before November's election. The market slid 6 percent between Oct. 1 and Nov. 15 in the run-up to the vote and immediately afterwards on concerns that Washington would be unable to enact reforms to keep the economy growing.

Evidence that growth is continuing, despite the political tensions in Washington, have kept stocks on an upward trajectory since then, leaving investors waiting for dips to add to their holdings.

"I'd love to have some sort of a pullback here because I'd think it's an opportunity," said Scott Wren, an equity strategist at Wells Fargo Advisors. "But it doesn't feel like we're going to have one in the near term."

The yield on the 10-year Treasury note, which moves inversely to its price, fell to 1.84 percent from 1.85 percent.

Markets were closed in observance of Good Friday last week. European markets were closed Monday for Easter.

Among other stocks making big moves:

? Tesla Motors jumped $6.04, or 16 percent, to $43.93 after the electric car company said sales are running ahead of schedule. The Palo Alto, Calif., company said Sunday night that first-quarter sales have exceeded 4,750 Model S sedans, above its previous forecast of 4,500.

? DFC Global, a finance company that provides loans to consumers without bank accounts, fell $3.60, or 22 percent, to $13.04 after slashing its earnings estimate for its fiscal year because of increasing loan defaults in its business in Britain.

? American Greetings rose $1.95, or 12 percent, to $18.05 after the company agreed to be taken private for about $602 million by a group led by some of its top executives.
