Weight gain after quitting smoking does not negate health benefits

Mar. 12, 2013 ? An analysis of data from the Framingham Offspring Study -- a long-term study that follows children of participants in the original Framingham Heart Study -- may have answered a question that has troubled individuals considering stopping smoking: do the health effects of any weight gained after quitting outweigh the known cardiovascular benefits of smoking cessation? The report in the March 13 issue of JAMA concludes that the benefits of stopping smoking far exceed any weight-gain associated risk.

"Among people without diabetes, those who stopped smoking had a 50 percent reduction in the risk for heart attack, stroke or cardiovascular death, and accounting for any weight increase didn't change that risk reduction," says James Meigs, MD, MPH, of the General Medicine Unit at Massachusetts General Hospital (MGH) senior author of the JAMA report. "In patients with diabetes -- among whom weight gain is a particular concern -- we saw the same pattern of a large risk reduction regardless of weight gained."

No study has previously investigated whether smoking-cessation-associated weight gain increases the risk of cardiovascular disease. One did look at the effects on risk factors such as blood pressure and lipid levels, but none have analyzed the actual occurence of cardiovascular events. Participants in the Framingham Offspring Study, which began in 1971, have a comprehensive medical exam and history taken every four to six years. The current investigation analyzed data from participant visits conducted from the mid 1980s into the mid-2000s, which covering the third to eighth visits for the overall study. The number of participants at each exam cycle ranged from almost 2,400 to about 3,250, totalling 11,148 individual person-exams.

Based on information gathered at each exam, participants were categorized as never smokers, current smokers, recent quitters -- who had stopped smoking since their last exam -- and long-term quitters. At the third study visit, 31 percent of participants were current smokers, and by the eighth visit only 13 percent continued to smoke. A general trend toward weight gain was seen across all study participants. Smokers, never smokers, and long-term quitters gained an average of 1 to 2 pounds between study visits, while recent quitters had gained an average of 5 to 10 pounds since their previous visit. But no matter how much weight they gained, the risk of cardiovascular events in the six years after quitting dropped in half for participants without diabetes. A similar drop in the incidence of cardiovascular events was seen in participants with diabetes, but it did not reach statistical significance, probably because less than 15 percent of the overall group was know to have diabetes.

"We now can say without question that stopping smoking has a very positive effect on cardiovascular risk for patients with and without diabetes, even if they experience the moderate weight gain seen in this study, which matches post-cessation weight increase reported in other studies," says Meigs, an associate professor of Medicine at Harvard Medical School.

Veronica Mars movie looks for crowdfunding, would have digital copies near release (video) (update: funded!)

There's been no shortage of movie projects that lean on crowdfunding to get production underway. Producing a movie based on a major TV series like Veronica Mars, however? That's fresh. Creator Rob Thomas has obtained support from Warner Bros. and lead actress Kristen Bell for a Kickstarter funding drive that, if it hits its $2 million goal, will shoot a full-length Veronica Mars feature this summer with a premiere around early 2014. Pledge makers would be rewarded with anything from a copy of the script through to a speaking role in the title. To us, the real highlight is the planned release strategy -- like an increasing number of movies, we'd be watching digital copies "within a few days" of the opening, rather than months. While Thomas and crew can't declare the fundraiser over until April 12th, it's advancing quickly enough that the focus is less on whether or not the project will go forward and more on its chances at becoming an internet-backed blockbuster.

Update: Just a few hours after kicking off its crowdfunding effort, the Veronica Mars movie project has surpassed its $2 million goal. Right now it's hovering around $2,035,000, but we suspect it'll balloon even further within the 30 days that remain in the campaign.

Humor gets the message out about colorectal cancer | TheGazette

The Iowa Department of Public Health is using a humorous approach to get people talking about a less-than-funny topic, colorectal cancer.

Humorous ?back side? drink coasters have been distributed to 26 bars and restaurants in 19 Iowa counties, including Beef O?Brady?s at 5420 Edgewood Rd. NE. in Cedar Rapids.

?We tried to think of a way to reach both men and women and it occurred to us that almost every small town has a tavern, small bar or restaurant where everyone gathers,? says Jeanna Jones, social marketing and education coordinator for the Iowa Get Screened program within the Iowa Department of Public Health.

?We thought we could get our message out by working through this different venue, enlisting the help of bartenders in telling people about the screening programs available, as well as raise public awareness about the importance of getting screened.?

Colorectal cancer is the?second leading cause of cancer deaths in Iowa. It often starts as a tiny growth or polyp in the colon or rectum and can be prevented if these polyps are detected and removed during a colonoscopy.

The front of each coaster approaches the hard-to-talk-about subject in a light, yet pointed, manner while the back side (no pun intended) provides key facts concerning colorectal cancer and information on how to get screened.

(AP) ? In a reunion with his former Boston teammates, Kendrick Perkins and the Oklahoma City Thunder got just the kind of victory he enjoys most.

Kevin Durant had 23 points and 11 rebounds, Russell Westbrook scored 15 points and Thunder beat the Celtics 91-79 on Sunday to stretch their winning streak to five games.

Boston was able to slow the Thunder from their best-in-the-NBA scoring pace of 107 points per game, but Oklahoma City's lowest output since Jan. 13 was still enough to come out ahead in this one.

"I told the guys coming into this game that it's going to be one of those low-scoring games, one of those nights that we have to grind out, so that's what we did," said Perkins, who won the 2008 NBA title with Boston. "Every night, we can't go out and expect to put up 100 points and the score to be 120-100. Some nights, it's going to be like this."

The Thunder limited Boston to 25 percent shooting in the second half after the Celtics shot 51 percent in the first. Boston dropped off to 33 percent in the third quarter and all the way down to 18 percent in the fourth, when the game got away.

"We were bad all game offensively," coach Doc Rivers said. "We were forcing things and offensively the ball stuck at times. Their length bothered us at times. Sometimes you want to win too bad, and that was us today."

Oklahoma City used an 11-2 run at the start of the fourth quarter to close it out. The Celtics had pulled within 68-65 after three quarters before going without a basket for the first 5 minutes of the fourth, and they never could recover.

Paul Pierce had 20 points to lead Boston, which had its own run of five straight wins broken. Kevin Garnett chipped in 10 points and 11 rebounds.

The Celtics finished at 38 percent shooting for the game with 19 turnovers, more than four above their average, while falling to 14-5 since All-Star Rajon Rondo's season-ending knee injury.

"We're showing that we can compete against pretty much anyone, but we have to bring our A-game most of the time, and that involves not turning the ball over and not fouling as much," said Pierce, who needs 10 points to pass Charles Barkley for 20th on the NBA's career scoring list.

It was the third-lowest scoring total of the season for Oklahoma City, which had 84 in its season-opening loss at San Antonio and 87 in a win at Portland in January. Boston even managed to outscore the youthful Thunder 11-8 on the fast break, but that slower pace still wasn't effective.

"This is what I like being a part of. All of that up and down fast pace, I kind of like the 'slow down, let's bump a little bit' type of game," Perkins said. "Either way, it's all good."

Jeff Green had a right-handed, reverse dunk as Boston closed the third quarter with six straight points and get within three. But the Thunder answered right back to start the fourth.

Backup point guard Reggie Jackson, who played at Boston College, got a running flip to go in and followed that with a double-clutch jumper from the right wing at the shot-clock buzzer.

When Jackson missed a more conventional shot, Nick Collison grabbed the offensive rebound and found a cutting Kevin Martin for a layup that turned into a three-point play and a 79-67 lead with 8:36 remaining.

"I was pretty upset with myself that it even came down to those type of shots, but I'll take them," Jackson said. "I guess I'll take them."

Martin added 12 points off the bench for Oklahoma City, which moved to 28-4 at home. The Thunder finish a run of four games in five days Monday night at San Antonio, with a chance to tie the Spurs ? without All-Star point guard Tony Parker ? for the best record in the Western Conference.

"It's important, no matter how guys try to look at it," Perkins said. "You always want home court as many playoff series as you can. We are going in there to try to get the win, and it's very important."

Notes: Green got a round of applause when he checked in midway through the first quarter, playing in Oklahoma City for the first time since being traded in February 2011. A few moments later, Jason Terry ? a member of Dallas' 2011 championship team that beat the Thunder in the West finals ? was booed, and he put his palms up, acknowledging the fans' grudge. ... When 38-year-old Derek Fisher checked in for the first time, Celtics coach Doc Rivers greeted him this way: "No retired players." ... The Celtics signed D.J. White to a second 10-day contract. Fellow reserve Shavlik Randolph is nearing the end of his 10-day deal. "We've had so much change over the last three or four weeks with different guys, I just like the continuity now," Rivers said. "I don't think there's anybody out there that's going to save the world. But both of those guys, I think, can help us in games."

Venezuela's Acting President Says Ch?vez's Body Will Be Permanently Displayed

The mourning over the death of Venezuelan President Hugo Ch?vez took a Lenin-eque turn today: Acting President Nicolas Maduro announced that his remains will be on permanent display at the Museum of the Revolution, "close to the presidential palace where Chavez ruled for 14 years," the AP reports.

The AP adds:

"Maduro says the president will lie in state first for at least another seven days.

"A state funeral for Ch?vez attended by some 33 heads of government is scheduled to begin Friday morning. Tens of thousands have already filed past his glass-topped casket at a military academy following a seven-hour procession on Tuesday which took his body from the hospital where he died."

SANDWICH ? Kevin Khater didn?t want to become another cog in the machine.

The doctor of radiation oncology said he decided early in his career to focus on rural cancer care.

?By me focusing on rural cancer care, I feel like I do touch people?s lives,? Khater said.

It?s to that end that he?s expanded his private practice to open the John Wentworth Cancer Center ? also known as SKAN LLC - Radiation Oncology ? in Sandwich.

Unlike a doctor in a large hospital or university setting, Khater said he feels more accountable by practicing directly in the community.

?I enjoy it. What I found is working in rural cancer care ? if I do a good job and go the extra mile and really do provide university quality ? the patients are very grateful,? Khater said. ?Practicing in rural America is unforgiving. If you do a bad job, people will know it. If you do a good job, people will know it. And I?m comfortable in that zone.?

Khater already has a private practice in Ottawa: Radiation Oncology of Northern Illinois. Helping him at his Sandwich location is his partner, Dr. Jyoti Kalra.

Khater said radiation oncology is one method in which cancer can be treated. Along with the other two methods ? surgery and chemotherapy ? radiation has its uses.

?We can treat an entire region and still preserve the normal anatomy,? Khater said. ?Radiation is good at controlling nonbulky diseases.?

The Sandwich location is named after John Wentworth, a two-term mayor of Chicago and a six-term congressman.

?Sandwich is a historical town,? Khater said. ?It prides itself on preserving history. We felt naming the center met the town?s philosophy.?

Khater went to school at Northwestern University and John Hopkins University. In addition to his medical degrees, he also has a doctorate in bio-physics.
