Healthcare-NOW! - Insurance Companies Gaming the System ...

Has your insurance company called you recently to ask you to sign up for a wellness or disease management program? Has that same company told you it?s a free service to policyholders and promised you that they do not share the information with the departments and people who administer your benefits and claims? You ? like me and millions of other Americans ? are being scammed.

Follow the money, my friends, and do not believe for one minute that the same company that would deny medications, health treatments and care to you would suddenly have your best interests at heart. It?s the money. It?s always been the money.

As soon as the Patient Protection and Affordable Care Act (PPACA) was passed, we were all told that finally the insurance companies would have to spend more of the premium dollars they collect on actual healthcare. Look it up here for the nitty-gritty.

In short, The Kaiser Family Foundation writes, ?The provision requires most insurance companies that cover individuals and small businesses to spend at least 80% of their premium dollars on health care (i.e. medical claims) and quality improvement, leaving no more than 20% for administration, marketing, and profit. Large group plans must spend at least 85 percent of premium dollars on health care. Insurers failing to meet these standards will have to pay rebates to consumers beginning later this year.?

And those rebate checks some policyholders are getting are also attributable to this provision of the Affordable Care Act. Sounds great, eh? What?s not to like about making the for-profit insurance companies spend more on actual healthcare or give us some money back?

What?s not to like is that never doubt for one instant that the for-profit insurance companies have found ways to subvert that well-intentioned ? or at least well-scripted ? nod to more accountability. Insurance companies like to make money ? it is what they are for and what they are all about.

Health insurance is not healthcare. Health insurance is a financial product sold to you and to me to protect our health and our wealth which may well do neither thing. Health insurance companies do not exist to make you healthy but to make their stockholders wealthy. If you think otherwise, you are not understanding the fundamentals of corporate life or the laws governing what corporate CEOs are to do for their primary customers ? their shareholders.

So, if required to provide more actual care that might reduce actual profits, what?s a good health insurance executive to do? Well, go to work changing what qualifies as actual healthcare under the new rules and then find ways to make even more money by providing that while denying more real healthcare services. And that?s exactly what is being done to you and to me.

Wellness and disease management programs now qualify under the ACA as part of what insurance companies can call ?medical loss? for purposes of meeting the criteria required to be spent on healthcare. Having that nurse or other insurance company employee call you and me monthly and recount which medications we take and for what conditions and then set an appointment to do it again every month is considered spending on health care. That?s right.

Just last week, on the same day my insurance company, Aetna, was denying medications ordered by my doctor during the diagnostic and early care phases of my latest cancer journey, I was being absolutely dogged by the Aetna Connection folks (their wellness and disease management plan) wanting me to reconnect with the nurse who ?needs? to engage with me on a monthly basis. Imagine, on one call I am begging Aetna to approve a medication needed to help me and relieve gut pain while on another call, I am telling the front person for Aetna Connection that I really don?t care about setting a new appointment with the wellness plan while Aetna is denying my care. I even told the caller I was groggy from the general anesthetic and that I couldn?t talk because of a sore throat from the intubation tube and that I hoped they?d call back another time when I could think more clearly, but she didn?t care. Like a collector of a very serious debt of some kind, she needed to get me assigned to a call date.

Imagine how many millions, if not billions, of dollars Aetna and every other insurer can now claim are being spent on healthcare through these programs. And imagine then how they will be actually able to deny real healthcare services when those millions spent on non-care figure into the totals they are required to spend on the new law.

We are screwed. At least I know I am. Aetna didn?t care for one moment if I got through this weekend in pain or was able to eat or rest comfortably. Aetna did care if I stayed in the Aetna Connection program. So, beware, fellow Americans. There is big money at stake in our healthcare system until it is changed to a more sane and honest system that values human life over cash. We need an improved and expanded Medicare for all for life system that cannot be gamed for profit.

Right now, we are still just part of the profit base for large health insurance companies and other healthcare corporations. We are not patients. Not yet. Follow the money.

Wall Street's Hottest Investment Idea: Your House | AOL Real Estate

Your house might be a better investment than you think. At least Wall Street seems to think so.

For a while now the conventional wisdom on real estate has been that while home prices might not fall much more, they aren't likely to go up anytime soon either. The best personal finance advice, then, when it came to buying a house, was to buy as little as possible.

Apparently, though, on Wall Street that common wisdom about home prices is not held by all, or even many. In the past six months or so, a number of investment firms, hedge funds, private equity partnerships and real estate investors have turned into voracious buyers of single-family homes. And not just any homes, but foreclosures. Investment banks, who also want in on the action, are lining up financing options to keep the purchases going.

Take for instance private equity mega-firm Blackstone Group. About a year ago, when The New York Observer profiled the firm's head of real estate, Jonathan Gray, there was no mention of single-family homes or even that the firm was looking to profit from a rebound in the residential real estate market.

Last week, Gray said that Blackstone now owns 2,000 single-family homes. At $300 million, that might be small compared to Blackstone's overall real estate portfolio of about $50 billion. But it's one of the biggest piles of homes ever intentionally put together by an institutional investor, and it's likely not the largest portfolio out there these days. (Banks and Fannie and Freddie are sitting on many more foreclosed homes, but that's a different story.)

Buying up single-family homes as an investment is nothing new. It's what landlords do all the time. But landlords have always tended to be mom-and-pop outfits often not owning more than a few dozen units confined to one area. Large Real Estate Investment Trusts and private equity funds generally focused on apartment buildings and commercial real estate, like malls and office buildings. That appears to be changing.

Kenneth Rosen, a professor at the University of California, Berkeley, who has a consulting firm that advises real estate investors, says that he knows of two dozen investment funds in the process of buying up single-family homes, a number of which are hoping to own as much as 10,000 homes around the country. He predicts that there could be as many as a dozen public real estate investment trusts, or REITs, in the next few years that are devoted to single family homes.

The funds aren't betting on home prices shooting up. The hope is to buy foreclosed homes at a discount, fix them up, and then rent them for a profit. But Blackstone's Gray says his firm's plan is to eventually sell the houses for a profit when the economy improves to the point where those who have become renters can afford to buy again, or when the banks really start lending again.

And that could look bad for Wall Street. Undoubtedly, to some this will once again be financiers making money off the bust that they helped create, especially since some of the people who are now lining up to buy foreclosed homes helped create some of the worst mortgage bonds, or bet against them. What's more, Harvard professor Matthew Desmond believes that Wall Street firms will band together and work to jack up rental prices on houses that they minimally maintain.

But without any evidence, it's probably a little too early to condemn Wall Streeters as slumlords. And no matter how many houses they buy up, fixing rental rates in the huge housing market will be much harder than manipulating Libor. What's more, the best thing that could happen for the economy right now is probably for the overhang of foreclosed homes to disappear, even if it doesn't immediately lead to rising prices. So this might prove to be an instance where Wall Street ends up doing what it's supposed to do, allocate needed capital to an undervalued sector of the economy, helping everyone in the process.

But perhaps the most interesting thing here is what this says about the value of your house. A report from Goldman Sachs earlier this year predicted that investors could generate 8 percent investment returns by buying up, fixing up and renting out homes. You have to pay for a mortgage. But still it probably means that buying a home right now or even owning the one you have, as long as you didn't overpay too badly, is a pretty good investment at a time when 10-year Treasury bonds are paying out 1.5 percent, and investment yield in general is hard to find. We could be finally planting the seeds of a rebound.

Dick Cheney recalls knowing his heart was 'at an end'

With a new heart, Dick Cheney is back.

In an exclusive interview with ABC News' Jonathan Karl, Cheney reflects on the "miracle" that a heart transplant promises to give him years of added life. And Cheney shows he is as confrontational as ever, taking on President Obama - and even Sarah Palin.

Cheney, 71, details how far back he has come. He remembers waking from weeks of heavy sedation after having a pump installed in his heart in 2010, a grueling operation that left him seeing his own mortality in the mirror.

"Two years ago this time I was on a respirator, heavily sedated. Just had a pump? installed on my heart because my heart had gotten so weak after six heart attacks and 30-some years of heart disease that it was, you know, it was at the end," he said.

"I lost 40 pounds. I was heavily sedated in the intensive care unit for weeks afterwards. I had pneumonia while I was in recovering from the surgery. And by the time I came out from under I looked in a mirror and what I saw was my dad shortly before he died. He was in his 80s," Cheney said.

That is why Cheney, in his first interview since his heart transplant four months ago, calls his recovery "nothing short of a miracle."

"I haven't felt this good in years," he said.

He doesn't know who gave him his new heart because protocol is to maintain anonymity, but the former vice president said there is a program that allows an intermediary to reach out to the donor family to see if they want contact with the recipient.

"At some point I would be, you know, certainly amenable to contact with the family. But we have not at this point exchanged any information," Cheney said.

If contact was made, Cheney said he would "express my gratitude for what's a magnificent gift. I can't think of a more magnificent gift than to be given additional years of life."

Healthy again, Cheney has resumed his love of fishing and his frank evaluations of politicians, including himself.

Cheney left office as one of the most unpopular vice presidents in the country's history, but the man known for his defiance of public opinion is vintage Cheney.

"I'm very comfortable with what I did, and why I did it, and how I did it," he said.

When asked if he had any regrets, Cheney replied, "Not really."

"You can't be judged, and shouldn't judge, people in those senior levels by the polls," he told ABC News.

He does not spare his fellow Republicans, calling Sen. John McCain's selection of Sarah Palin as his vice president running mate a "mistake."

Cheney said Palin had been governor of Alaska for two years at the time and "did not pass the test" of being ready to be president.

He also wryly described how the list of vice presidential candidates is used by officials up for reelection.

"I had a couple of calls from politicians who'd say, 'You know, it'd really help me in my race back home, Dick, if I was on the list.' Done. You're on the list," said Cheney. "Then somebody could go leak the fact that they were on the list."

Cheney helped vet candidates for Gerald Ford in 1976 and George W. Bush in 2006 that ultimately settled on Cheney.

"But that was the big list. It was easy to get on the big list. The tough part is the small list, the one that's really under active consideration," he said.

Cheney's harshest criticism, however, was for President Obama.

"I obviously am not a big fan of President Obama. I think he's been one of our weakest presidents. I just fundamentally disagree with him philosophically. I'd be hard put to find any Democratic president that I've disagreed with more," he said.

When asked if he thought Obama was "worse than Jimmy Carter," Cheney replied, "Yes."

Cheney gave Obama credited for killing al Qaeda leader Osama bin Laden, but added that "a lot of that intelligence that laid the groundwork for what ultimately led to the capture of Bin Laden [was] as a result of programs we had in place in the Bush administration."

He was sharply critical, however, of Obama's plans to withdraw troops from Iraq and Afghanistan.

"We should not be running for the exits. We should not be turning our backs on our friends in that part of the world," he said.

Uganda bans physical contact as Ebola reaches capital

Uganda's president Monday warned against physical contact after a victim of a deadly outbreak of the Ebola virus was reported in the capital Kampala for the first time.

"The Ministry of Health are tracing all the people who have had contact with the victims," Yoweri Museveni said in a state broadcast, adding that 14 people had died in total since Ebola broke out in western Uganda three weeks ago.

Two cases have since been reported in the capital, with one victim reported dead in Kampala's Mulago Hospital, he said, calling on people not to shake hands to avoid the spread of the killer virus.

"Ebola spreads by contact when you contact each other physically... avoid shaking of hands, because that can cause contact through sweat, which can cause problems," Museveni said.

"Do not take on burying somebody who has died from symptoms that look like Ebola -- instead call health workers because they know how to do it...avoid promiscuity because this sickness can also go through sex," he added.

Seven doctors and 13 health workers at Mulago hospital are in quarantine after "at least one or two cases" were taken there, with one later dying from the virus.

"I appeal to you to first of all report all cases which appear to be like Ebola, and these are high fever, vomiting, sometimes diarrhoea, and with bleeding," he added.

"When you handle this case well you can eliminate Ebola quickly."

According to experts, despite being extremely virulent, the disease is containable because it kills its victims faster than it can spread to new ones.

It has a fatality ratio of between 23% - 90%, according to the World Health Organisation (WHO).

The latest outbreak started in Uganda's western Kibale district, around 200 kilometres (125 miles) west of the Kampala, and around 50 kilometres from the border with Democratic Republic of Congo.

What are the epistemological underpinnings and rationale for the use ?digital? methodologies? First, be aware of the food you have. Maybe keep a little magnetized dry erase board on the refrigerator door so you can jot down the leftovers that you?ve put in it through the week (don?t forget to date them, too).Second, don?t let leftovers exist beyond the 4 day mark. If there is to be a carbon tax, it should be narrowly focused on pollution rather than an excuse to expand government control over the economy or hand money to politically favored groups.

And that's just the beginning of the list. Be alert to these often-sneaky causes of acne:

Makeup. Acne cosmetica, or pimples caused by topical creams, lotions, and makeup, is most common on the face, neck, hairline, and scalp. Products that contain mineral oil clog pores, so switch to brands labeled non-comedogenic, which are oil-free, don't strip skin of necessary moisture and nutrients, and don't block pores. And since makeup brushes collect bacteria and yeast, clean them once a week with soap and water. Washing that foundation and powder off every night is important, too, says Francesca Fusco, a dermatologist based in New York. She recommends using exfoliating wipes and pads, which are gentle but remove more dead skin than plain-old soap does.

Cell phones. Your iPhone or Android is a cesspool of dirt and bacteria, and there's a good chance it'll trigger breakouts on your chin and around your mouth. Wipe it down daily with alcohol or Clorox wipes to keep it clean.

Medication. Check your medicine cabinet and tread particularly carefully with prescriptions that emphasize steroids, such as prednisone, cortisone, and hydrocortisone. These can both aggravate and cause acne. Also watch out for lithium, lithium chloride, and certain forms of iodine. "Unfortunately, many of these medications are given for serious conditions, and there are few available substitutes," Krant says. "So people can get stuck in troubling situations, having to take important medication that causes distressing side effects." Keep an open line of communication with your doctor, and make sure he's aware of your concerns.

Yoga mats. Go to class, grab a mat ... and consider all those who have come before you. Touching your face. Beware of contact acne: "It can appear anywhere you apply repeated or extended pressure on your skin," says dermatologist Ava Shamban, author of Heal Your Skin. "Resting your chin on your hand while you work, pressing your cell phone on your chin, or wearing tight clothing can all contribute to acne." She recommends trying not to touch your face, switching to a headset, and opting for a ponytail holder instead of a headband. And during the summertime, don't sit down in a wet bathing suit: Doing so puts you at risk of developing butt acne.

Dandruff. Dead skin blizzard? Not fun, for plenty of reasons. And it doesn't help that dandruff often causes acne, typically in the form of tiny pimples lining the scalp. Avoid sunscreen with perfume or dyes, and steer clear of spray-on formulas; many of these contain alcohol, which may irritate sensitive skin, Shamban says. Products made with zinc oxide are often more tolerable than those with titanium dioxide.

Traveling. Changes in environment--humidity, weather, and even minerals or fluoride in water--can trigger breakouts. There's nothing you can do about the heat index, of course. But wash your face with bottled water when you can, and avoid using hotel soaps.

Stress. It triggers heightened levels of androgens, hormones that contribute to adulthood breakouts. Stress also releases cortisol and other adrenal steroids that can stimulate the sebaceous glands and lead to acne flare-ups, Shamban says. Though it doesn't always cause new cases of acne, stress tends to worsen matters in those already struggling with the condition. Make sure to get enough sleep, and allot 15 minutes each day to relaxing or doing something you enjoy. Squeeze in some exercise, too, since research suggests it helps deflate stress.

Hair-styling products. It's called pomade acne: Breakouts caused by hair gel or any other styling staple. These cause oil to seep onto your forehead, trapping acne-causing bacteria in your pores. Apply products with your hands, keeping them away from the hairline, and wipe your skin with facial cleanser to remove any remaining traces. Be wary of bangs, too, since they bring hair products directly against your forehead.

Chin straps. Bikers and athletes who wear helmets are prone to this type of acne, caused when straps rub against the skin. "Wash your face before and after putting it on, and keep the strap clean," Fusco says. She also recommends using an antibacterial pad to wipe straps down before and after use.

Anti-aging creams. Think you're doing your skin a favor? Think again. Many of these contain retinol, which stimulates cell turnover, increasing the number of acne-like lesions. "Too much turnover can cause a traffic jam in the skin layers," Krant says. Though Retin-A, which contains retinol, is sometimes prescribed to treat acne, experts warn that many people find it to be a skin irritant; plus, anti-aging creams tend to be oily. "Always use these sparingly, and ideally, under a dermatologist's instruction."

UK stocks extend falls; earnings provide direction

LONDON July 25 (Reuters) - Britain's top share index eased

back on Wednesday, extending recent sharp falls into a fourth

straight session, tracking declines on Wall Street and in Asia

as worries that Spain may need a full debt bailout continued to

dog global markets.

The main early direction, however, was dictated by a batch

of blue chip earnings, with BT Group and British American

Tobacco big fallers as their latest updates failed to

impress, while British Land and ARM Holdings

got a boost from well-received results.

At 0708 GMT, the FTSE 100 index was down 16.51

points, or 0.3 percent at 5,482.72, having closed down 0.6

percent on Tuesday to end below 5,500 level for the first time

since June 28.

Chelsea signs Brazilian playmaker Oscar

LONDON (AP) - European champion Chelsea has signed Brazilian playmaker Oscar from Internacional in a deal said to be worth 25 million pounds ($38.7 million).

The 20-year-old, whose full name is Oscar dos Santos Emboaba Junior, currently is with the Brazilian team for the London Olympics. The transfer was announced Wednesday.

Oscar made his debut for Brazil's national team as a substitute in a 0-0 tie with Argentina last September. He has one goal in his six international appearances.

Oscar is a product of the Sao Paulo youth academy and joined Internacional in December 2010.
